Request for Temporary Change of Course Modality
August 23, 2021
Georgia State is an institution of the University System of Georgia (USG). As a unit of the USG, it is required to follow policies and directives set by the central system.
The USG has made its policy clear: temporary modality changes may only be made where the instructor tests positive or has a need to quarantine and is able to teach remotely. In these circumstances, the instructor must obtain the approval of the dean and Office of the Provost to temporarily change the course modality. The change must not extend beyond the period of quarantine.
The university has created an online form to submit and route these requests for review. Instructions for submission and the form link are available by selecting the DOWNLOAD button below.
